About the Program

Our residential programs include intensive stabilization & rehabilitation programs, as well as community residences. Each provides quality residential services and care to individuals seeking to address their chemical dependency problems. Individuals appropriate for residential services also participate Øin outpatient treatment services and/or other support services such as vocational or educational services.

Our Community Residences are Jones & Alexander (for men) and Barrington (for women)

What the Program Offers

Services include housing in a supportive, recovery oriented community, relapse prevention group support, individual counseling support, case management, vocational/educational development, transitional services development, addiction education and Life Skills Classes including Anger Management and Relapse Prevention and recreational, sober, fun activities.

Eligibility criteria

Adults 18 years or older who meet the criteria established by NYS OASAS level of care determination for CD Community Residential Services, indicating a need for a supportive residential environment to sustain recovery.

Intake Process

Fax a copy of the Monroe County Universal OASAS Residential Referral Form to the Intake Coordinator (fax number, below), along with any other identified information regarding eligibility for the service. Our Intake Coordinator will review documentation and confer with the client and other referral contacts for placement. A visit to the residence for dinner, brief orientation and meeting with the community members is scheduled. Following this visit if the individual is still interested, they are contacted when a bed becomes available. A date is finalized for the individual involved to enter treatment and immediate needs are assessed.

Program costs may be covered through Public Assistance Benefits, paid separately through Supplemental Security Income payments or as determined in a sliding fee scale arrangement.

About the Program

Our residential programs include both intensive residential stabilization and rehabilitation programs, and community residences. Our intensive residential programs are for adults ages 18 years and older, including options for women who are pregnant and parenting young children, preschool age and younger. There are also children’s beds for preschool or infants to reside with their mothers.

Our residential programs are 24/7 structured treatment and recovery services focused on providing both stabilization and rehabilitative treatment. Services provided to persons recovering from substance use disorders are centered around individual therapy, group therapy, case management, job skill development and anger management training. Onsite primary medical care, nursing services, psychiatric assessment and mental health counseling are also incorporated. Our residential programs also collaborate with resources in the community and the integrate use of community supports and programs.

Our residential programs for men and women recovering from addiction include provide structured treatment, recovery services, care, and housing. These supportive communities help prevent relapse and include individual and group therapy, case management, job skill development, and anger management training. Onsite medical care, nursing services, psychiatric assessment and mental health counseling further support stabilization and rehabilitation.

Our Intensive residential drug treatment programs include Freedom House (for men); Hannick Hall (for women with preschool age children, Wayne Co. and surrounding area) & Liberty Manor (for women with children).

What the Program Offers

Services include housing in a supportive and highly structured, recovery oriented community, individual and group therapy, addiction education, relapse prevention group, case management, fatherhood classes, vocational/educational development, transitional services development, Life Skills Classes including Anger Management and Relapse Prevention and recreational, sober, fun activities. Some programs also incorporate onsite primary medical care, nursing services, psychiatric assessment and mental health counseling to support those needing addiction, medication and mental health services. Prevention and wellness activities supplement the treatment services we offer.

Eligibility criteria

Adults 18 years or older who meet the criteria established by NYS OASAS level of care determination for Chemical Dependency Intensive Residential Services, including: a chemical dependency diagnosis, a need for a highly structured residential environment to sustain recovery, requiring assistance with development of social support and life skills to develop and sustain recovery. Pregnant women and women with young preschool age children may also be in residence with their children.

To help patients suffering from a substance use disorder find facilities that deliver quality treatment and care, Excellus BlueCross BlueShield recognizes Family and Community Services Restart Liberty Manor as one of the first facilities to receive the Blue Distinction® Center for Substance Use Treatment and Recovery designation – a new designation under the Blue Distinction Specialty Care program.

Blue Distinction Centers are nationally designated facilities that show a commitment to delivering improved patient safety and better health outcomes, based on objective measures that were developed with input from the community and leading accreditation and quality organizations.

Blue Distinction Centers (BDC) met overall quality measures, developed with input from the medical community. A Local Blue Plan may require additional criteria for providers located in its own service area; for details, contact your Local Blue Plan. Blue Distinction Centers+ (BDC+) also met cost measures that address consumers’ need for affordable healthcare. Each provider’s cost of care is evaluated using data from its Local Blue Plan. Providers in CA, ID, NY, PA, and WA may lie in two Local Blue Plans’ areas, resulting in two evaluations for cost of care; and their own Local Blue Plans decide whether one or both cost of care evaluation(s) must meet BDC+ national criteria. National criteria for BDC and BDC+ are displayed on www.bcbs.com. Individual outcomes may vary. About the Program

Supportive Living is a community based apartment program providing residential services to men and women needing support to maintain abstinence and develop life skills to maintain recovery. Along with this assistance, clients readapt to independent living in the community while receiving any needed clinical and peer support through other community agencies.

What the Program Offers

Services include: housing in a recovery oriented, community based apartment setting, support for ongoing development and monitoring of life skills, peer recovery support, referral to vocational/educational services, monthly community meetings, quarterly family sober fun activity, case management support, bi-weekly drop-in visits, assistance in transition to independent living.

Eligibility criteria

Adult males or females 18 years or older who meet the criteria established by NYS OASAS level of care determination for Supportive Living Services, indicating a need for a supportive residential environment to sustain recovery.
